There is a rapid increase in the emergence of multidrug resistance which consider as one public health threats all over world. MDR was defined acquired non-susceptibility to at least agent three or more antimicrobial categories. The present study aims determine dominant multi-drug resistant bacteria five biggest hospitals Asir region, Saudi Arabia. Antibiotic-resistant isolates were collected from region between 30 March and 1 June 2018. tentatively identified by routine bench tests confirmed using VITEK® 2 Compact. antibiotic susceptibility these latest instrument. Routine Compact 12 bacterial strains including; Staphylococcus aureus, haemolytics, epidermidis, Streptococcus pneumoniae, Enterococcus faecium, Enterobacter cloacae, Acinetobacter baumannii, Proteus mirabilis, Klebsiella Pseudomonas aeruginosa, Escherichia coli penneri. P. aeruginosa majorly found selected hospitals. Antibiotic this showed that highest rate against penicillins (21.6%) cephalosporin (20.1%). These have shown fluoroquinolones (13.8%) aminoglycosides (10.1%). concluded high β lactam antibiotics, particularly cephalosporins, considered new threat
